Board Vacancy Application This application is for township residents who are interested in serving on a township board.  The application will remain on file for one year from the date it is received.

Landlord Report Form This application is to be submitted by Landlords to report the address of rental properties and the names of tenants 18 years and older occupying the rental property.  Any changes in tenancy are to be reported within 60 days.

Request/Complaint Form This application is submitted to file a request or complaint.  The application will be forwarded to the Board of Supervisors for review.  This form is considered a township record and is available for public inspection under the Right To Know Law.
